Coaching Guidelines: Welcome! Please make yourself familiar with these guidelines as we enter into this positive and sacred coaching partnership! Terms of Coaching Fees: My Coaching fee is payable to Coaching with Patricia, LLC. Procedure: I will call you at our appointment time. Changes: If you need to reschedule our appointment, please give me 24 hours notice. Between Calls: Email me any time at patricia@coachingwithpatricia.com if you ever need me between coaching sessions. Problems: Please let me know if you are ever uncomfortable or have any concerns! Coaching Agreement Coach I agree to serve as your Coach - to partner with you help to reach your personal and professional goals. As your Coach, I do not guarantee results. Results come from having the courage to take informed action in your own life. I am not a physician or therapist and I am not offering medical or therapeutic advice of any kind, and not treating any medical conditions. I will honor your privacy to the fullest extend of the law, so long as I don't fear for your own or another's safety. Client I am committed to moving forward in positive action in my life. I accept full responsibility for myself and any actions I take that might result from Coaching. I have consulted with a physician and /or therapist as appropriate about my intention to engage in coaching and am healthy enough to engage in coaching. I can afford the Coaching fee and agree to pay for the services I use. I agree to attend my scheduled sessions. I have read and agree with the Coaching Guidelines and the Coaching Agreement.
